Abstract: | This evidence review was conducted to explore the effectiveness of wellbeing tools that assess the needs of patients, clients and service users in times of change. This included the efficacy of existing tools to identify change needs, those that provide monitoring information to chart progress and those that can be used as a motivational tool. The recommendations from the evidence review will be used by First For Wellbeing to inform their community-based health and wellbeing initiatives, including the development of their own wellbeing tools. |
